How to Create a Survey in Canvas

 

Introduction

Surveys allow instructors to collect student feedback or responses in Canvas with or without impacting course grades. Surveys are created using Quizzes.

Step-by-Step

  1. In your Canvas course, select Quizzes from the Course Navigation menu.

  2. Click the +Quiz/Survey button

  3. Enter the survey title in the Assignment Name field.

  4. Select the caret to access the Quiz Type dropdown menu.

  5. Select Graded Survey or Ungraded Survey, depending on your preferences.

  6. If desired, select Keep Submissions Anonymous to prevent responses from being associated with student names.

    NOTE:⚠️ Once anonymity is enabled, it cannot be removed.
     

  7. Adjust additional settings as needed, such as time limits or availability dates.
     

  8. Then, select the appropriate Assignment Group for the survey from the Assignment Group dropdown menu.
     
     
  9. In the Assign section, choose who should complete the survey.

     

  10. Set a Due Date, Available From, and Until dates if desired.

     

  11. Select Build.

Add Survey Questions

  1. In Build

  2. Select + Add button to create questions.

  3. Choose a question type (Multiple Choice, Essay, etc.).

  4. Enter the question text and response options.

  5. Repeat for additional questions as needed.


Step 6: Save and Publish

  1. Click Return to save your survey.

  2. Make sure to Publish the survey to make the survey available to students.


 

Notes

  • Anonymous surveys prevent instructors from seeing which student submitted each response.

  • Survey responses can be reviewed after submissions are received.
