Within Survey Builder you can custom create a new survey/form from scratch.
Step-by-Step:
- Click on Create.
- Click on Create a New Survey
- Click on Create a New Survey
- Your Contact information will be auto-filled
- Enter a Title for Your Survey
- Your Contact information will auto-populate
- Enter Instructions for the survey if any
- Make Active
- Click Save Form
- You will now be taken to the Questions tab. This is where you will be adding the questions for your survey. Click on Add
- Select the Question Type by clicking on the drop down arrow next to Question Type
- Fill out the question type that you selected, and be sure to complete the settings area under the question. Each question type will have different setting options
- Click on Save Question
- Repeat steps 6-8 until you have added all questions needed
- Click on Access
- Edit the Open and Close dates if needed
- Click on Preview, to view the survey
- Click on Survey details to get link to survey to distribute for completion
