Within Survey Builder you can custom create a new survey/form from scratch. 

Step-by-Step: 

  1. Click on Create.

  1. Click on Create a New Survey 
  2. Click on Create a New Survey

  1. Your Contact information will be auto-filled

  1. Enter a Title for Your Survey
  2. Your Contact information will auto-populate
  3. Enter Instructions for the survey if any
  4. Make Active
  5. Click Save Form
  6. You will now be taken to the Questions tab.  This is where you will be adding the questions for your survey.  Click on Add survey add.png
  7. Select the Question Type by clicking on the drop down arrow next to Question Type question type text drop down highlighted.png
  8. Fill out the question type that you selected, and be sure to complete the settings area under the question.  Each question type will have different setting options
  9. Click on Save Question
  10. Repeat steps 6-8 until you have added all questions needed
  11. Click on Access
  12. Edit the Open and Close dates if needed  
  13. Click on Preview, to view the survey 
  14. Click on Survey details to get link to survey to distribute for completion
